For the duration of our sessions I will ask you for some information about you such as:
1. Personal details including name, address, date of birth, telephone number, email address. This is so I can contact you and identify you if I need to make emergency phone calls.
2. Next of kin and emergency phone number. In case of emergency.
3. GP's name, address and phone number. Contact with your GP will only be made with your consent and will be in case of emergency or for onward referral if discussed between us.
Your personal data will not be passed to any third party without your consent and will be removed from my records at the termination of our sessions.
I am required to keep brief anonymised notes of our sessions, and to keep these for a period of 7 years, after which time I will destroy them by shredding.
Your signature indicates that you have given me your personal data voluntarily and that you understand how I will look after any information you have passed to me.
My contract and privacy agreement will be given to you for signature at the commencement of your sessions.
